29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about June 14, 2018 and at times prior thereto, the drill press located in the vicinity of the tool cage was operated without a guard. This condition exposed employees to pinch point hazards at the point of operation.

29 CFR 1910.215(a)(1): Abrasive wheel(s) used on grinding machinery were not used with safety guard(s): On or about June 14, 2018 and at times prior thereto, the four and six inch portable grinders located in the tool locker were not equipped with guards. This condition exposed employees to amputation hazards.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: On or about June 14, 2018 and at times prior thereto, the portable fire extinguishers that were mounted on the wall in the vicinity of the drill press and the bay doors had no signage identifying them as the location of the fire extinguishers. This condition exposed employees to burn hazards in the event a fire extinguisher needed to be rapidly identified and used.
