29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).    Location - 7 Progress Avenue, Chelmsford, MA.    On or about June 1, 2018, employer did not conduct frequent or regular inspection of the job site that would have identified the following hazards:    Instance one. Open manhole cover that an employee fell in to and suffered an injury(s).    Instance two. A scaffold that was not made by a competent person. Employees were working/walking on a scaffold made out of wood planks and pallets.    Instance three. Tubular scaffold section being used by employees to ascend and descend scaffold made out of wood planks and pallets.
