29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about June 15, 2018 at the Courtyard Marriott hotel, located at 10307 N. Mall Drive, Baton Rouge Louisiana, The employer allowed employee(s) to preform roofing activities without proper fall protection. The approximate distance from the roof eave to the ground was approximately twenty eight feet. The pitch of the roof was 5 in 12. The measurement was given to the CSHO my Mike Busch (General Contractor).

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  On or about June 15, 2018 the employer did not ensure the employees conducting roofing activities at the Courtyard Marriott Hotel located at 10307 N. Mall Drive, Baton Rouge, Louisiana were trained and had written certification which enabled the employee(s) to recognize fall hazards and the procedures to be followed to minimize those hazards.
