29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: Jobsite: On or about June 12, 2018 and at times prior thereto, the employer failed to protect employees from the potential respiratory hazard of nuisance dust in the shop area through the voluntary use of N-95 respirators and other tight fitting, elastomeric respirators without providing the basic advisory information in Appendix D of 29 CFR 1910.134.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.: Jobsite: On or about June12, 2018 and at times prior thereto the employer did not ensure that air used for cleaning was reduced to less than 30 p.s.i. exposing employees to struck-by hazards while employees were cleaning the tops of the laminated sheets of plywood before putting the sheets in the press machine.
