29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:  Building No. 1:  On or about June 15, 2018, employees were exposed to a fall hazard greater than 6 feet, while installing sheets of plywood decking on a roof, without using personal fall arrest systems, or alternative fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to a fall hazard that enabled each employee to recognize the hazards and procedures to be followed in order to minimize these hazards:  Building No. 1:  On or about June 15, 2018, employees were exposed to a fall hazard while installing sheets of plywood decking on a roof without being effectively trained in fall protection measures.
