29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  (a) Lopez, Selvin Orlando located at 1169 Picard Lane, Fort Collins, CO 80526: On and preceding 5/14/2018, the employer did not ensure that employees were protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects.  Employees working on different levels of a residential roof were not utilizing head protection.  The employees were installing shingles and handing other materials on the two levels of the roof which put one employee holding materials above the other employee without head protection utilized.  This exposed the employees to struck by hazards.

(a)  Lopez, Selvin Orlando located at 1169 Picard Lane, Fort Collins, CO 80526: On and preceding 5/14/2018, the employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles to include but not limited to debris from use of pneumatic nail guns.  Employees installing shingles on a residential roof were not utilizing eye or face protection.  This exposed the employees to struck by hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a) Lopez, Selvin Orlando located at 1169 Picard Lane, Fort Collins, CO 80526: On and preceding 5/14/2018, the employer did not ensure that employees engaged in residential construction activities 6 feet or more above lower levels were protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed;  (a)  Lopez, Selvin Orlando located at 1169 Picard Lane, Fort Collins, CO 80526: On and preceding 5/14/2018,  The employer did not ensure that ladders used to access an upper landing surface had side rails extended at least three feet above the upper landing surface.  Employees were using and extension to access the roof with approximately 1' extending above the landing surface.  This exposed employees to falls from height.
