29 CFR 1926.403(b)(1): The employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:   In the alternative:  29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  (a) This violation occurred on or before May 22, 2018, at the job site located at 3100 Northside Blvd, Richardson, TX 75080, at the Northside II Apartment Complex, building #6, on the fourth floor, where employees working and installing electrical wires to the Schindler model 3300 simplex elevators exposed to an electrical hazard.  (b) This violation occurred on or about May 22, 2018, at the job site located at 3100 Northside Blvd, Richardson, TX 75080, at the Northside II Apartment Complex, building #6, on the fourth floor, on the Schindler model 3300 simplex elevator, where employees working, testing, adjusting, balancing, and connecting auxiliary wires were exposed to an electrical hazard.
