29 CFR 1910.140(c)(22): Personal fall protection systems were not worn with the attachment point of the body harness located in the center of the employee's back near shoulder level:      a) Job site, at/near 48 26th Street, Pittsburgh, PA: On or about June 21, 2018, during window washing activities, the employer did not ensure that an employee, working in an aerial lift at heights of over 3 stories, was using fall protection systems appropriately in that he was using a fall arrest system and was tied off in the front of his harness at waist level rather than on the D ring on his upper back.
