29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:       a) North side of building, on or about 6-19-18:  Employees were using a fiberglass extension ladder for access to a roof, with side rails that were 16 inches above the landing surface, exposing the employees to a fall hazard of  22 feet.       b) East side of building, on or about 6-19-18:  Employees were using an aluminum extension ladder for access to a roof, with side rails that were 14 inches above the landing surface, exposing the employees to a fall hazard of  23 feet 6 inches.
