29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about June 20, 2018 at 105 Towne Lake Parkway, Woodstock, Georgia, employees in the process of removing the old roof and installing a new one were exposed to a fall hazard of more than six (6) feet to the lower level.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person. On or about June 20, 2018 at 105 Towne Lake Parkway, Woodstock, Georgia, employees were exposed to a fall to the lower level when a nail was used to attach a rope to the roof.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about June 20, 2018 at 105 Towne Lake Parkway, Woodstock, Georgia, employees using extension ladders to access the working level were exposed to a fall hazard when the ladder did not extend three (3) above the landing.
