29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about 6/17/2018, The employer failed to ensure that machine guarding methods were installed and utilized on the following machinery:    a) #217 Motch T25VNC,  a new employee was being trained when he/she reached inside the Machine while the clamps were engaged,    b) #218 Motch T25VNC, , the door was able to close after momentarily tapping the green cycle button creating a pinch point between the door and the frame. The button is about 4" from the pinch point, and    c) The control panels on #217 Motch T25VNC, and #218  Motch T25VNC, were on a swivel that allowed the control panel to be moved within a few inches of the doors.
