Werner aluminum extension ladder to access the roof and the ladder side rails were extended approximately 6 inches above the upper landing surface, exposing employees to injuries from a fall or tipping hazard, observed on or about June 22, 2018.  b) Jobsite: Employees were using a 30 ft. Werner extension ladder to access the roof and the ladder side rails extended approximately one foot above the upper landing surface, exposing employees to injuries from a fall or tipping hazard, observed on or about June 22, 2018.  c) Jobsite: Employees were using a 30 ft. Werner extension ladder to access the roof and the ladder side rails extended approximately 2 feet above the upper landing surface, exposing employees to injuries from a fall or tipping hazard, observed on or about June 22, 2018.
