29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: On or about 6/20/2018, during bricklaying activities located at Acme Fresh Market, 3200 Greenwich Rd., Norton Ohio, the employer failed to ensure that employees were protected from fall hazards by installing top rails and mid-rails on the supported scaffolding system.
