HSM Solutions, Inc., Fort Valley, GA - On or about June 22, 2018, and times prior to, the means of egress pathway to an exit was reduced to less than the 28 inches.  During an emergency situation, employees working in passage ways, adjacent to this equipment, would be exposed to fire hazards.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

The employer allowed bus seats and other equipment to remain in the aisles, creating an egress hazard. 29 CFR 1910.37(b) (4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:  HSM Solutions, Inc., Fort Valley, GA (Throughout Facility) - On or about 6/22/18, and times prior to, exit routes signs, and doors were blocked with fixed storage equipment such as, but not limited to shop material/boxes, and racks of bus seats.  Employees were exposed to entrapment hazard. There were no directional exit markings indicating the way to travel, exposing employees to an entrapment hazard.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

Employees have to change the propane fuel forklift and/or carry 60 pound seats without adequate personal protective equipment or training. 29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes:  HSM Solutions, Inc., Fort Valley, GA - Throughout the facility - On or about June 22, 2018, and at times prior to, employees changed out propane tanks on forklifts without the use of protective gloves for propane.  The employer was not providing or requiring employees to use propane resistant gloves.  Employees were exposed to hand injuries such as but not limited to frost bite. An employee was observed lifting two racks of bus seats above the head of the driver and the load was unstable and shaking.
