On or about 21 June 2018, and at times prior thereto, four masons were performing masonry work on a scaffold without the use of hard hats, exposing the masons to the hazard of being struck by falling tools, materials, and debris.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii). On or about 21 June 2018, and at times prior thereto, a mason was performing masonry work on a scaffold at the second tier in which the platform was not fully planked from front to back, exposing the mason to the hazard of falling approximately 11 feet 7 inches to the ground below.

On or about 21 June 2018, and at times prior thereto, four masons were performing masonry work on a scaffold at the third tier without the use of fall protection, exposing the masons to the hazard of falling approximately 18 feet to the ground below. JYI Masonry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(4)(I), which was contained in OSHA inspection number 1289193, citation number 01, item number 001 and was affirmed as a final order on 03/30/2018, with respect to a workplace located at 903 William D. Fitch Pkwy., College Station, TX 77845.
