This cord was visibly damaged and exposed the employees to electrical hazards. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

Crossbraces were used as a means of access. a- 175 New Egypt Road, Lakewood NJ: On or about 06/25/18, company employees were exposed to fall hazards in excess of 14 feet to the ground surface below when accessing and egressing the working levels of a supported scaffold using the scaffold's crossbraces.
