29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of �1926.502(k) developed and implemented:                                                                                                             The employer is failing to ensure that employees are protected from fall hazards. This was most recently documented at 3915 S. 208th St., Elkhorn, Nebraska 68022. Employees were wearing harnesses without lanyards while working on the roof of a home which was approximately 16 feet from a lower level.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed.
