The employer is failing to protect employees from struck-by hazards associated with using pneumatic nail guns without wearing approved eye protection.  The most recent occurrence of this was observed on June 26th, 2018 at an existing residential home located at 13103 Himebaugh Ave, Omaha, NE where the employer has employees using pneumatic nail guns to secure the shingles they were installing on the roof without wearing any ANSI approved eye protection.

The employer is failing to protect employees from fall hazards associated with performing residential construction activity while exposed to falls 6 feet or greater.  The most recent occurrence of this was observed on June 26th, 2018 at an existing residential home located at 13103 Himebaugh Ave, Omaha, NE where the employer has a crew of 4 employees installing roof shingles exposed to fall hazards while they were working off the roof of the home without any form of conventional fall protection.
