29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system:    On 06/25/2014, an employee(s) from Matt Rowles Construction was exposed to a fall hazard while working from the second floor window areas of a residential structure located at Lot #1 in Aspen Meadows, Cottleville, MO.  The employee(s) was not utilizing any type of fall protection (personal fall arrest system).  This employee was exposed to a fall to the ground below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On 06/24/2014, an employee from Matt Rowles Construction was exposed to a fall hazard while working from the front porch roof structure.  This employee was exposed to a fall to the ground below.    The Matt Rowles Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number1256556, citation number 1, item number 1, and was affirmed as a final order on October 2, 2017 with respect to a workplace located at Lot 88 Brookfield Crossing Drive, Wentzville, MO.
