29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  On or about June 26, 2018, employees performing residential roofing operations were exposed to struck by hazards when employees worked on two different levels when the employer failed to mandate the use of hard hats.    No abatement certification or documentation is required for this item.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   On or about June 26, 2018, employees performing roofing installation operations on the upper level of a single-family property were exposed to a 18.9 foot fall when the employer failed to ensure that fall protection was utilized.   No abatement certification or documentation is required of this item.
