29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments: a. The Sheridan Building located at 125 South 9th Street, Philadelphia, PA 19107 - The employer did not trained employees in safety-related work practices, including but not limited to, when and how to de-energize electrical equipment, the hazards of arc-flash, the purpose of electrical warning signs and labels, protective grounding, and general electrical hazards. Employees work duties included but not limited: basic electrical work, such as, the replacement of lights and lighting ballasts for fluorescent lights, and checking if junction boxes, control boxes, connections and other wiring have covers. Observed On or About 06/22/18

29 CFR 1910.303(h)(2)(iii)(B): The employer did not mark metal-enclosed switchgear, unit substations, transformers, pull boxes, connection boxes, and other similar associated equipment with appropriate caution signs; and danger signs: a. Sub-basement, The Sheridan Building located at 125 South 9th Street, Philadelphia, PA 19107 - The employer did not affix Arc Flash hazard warning labels to a cubicle that contained a high voltage Air Interrupter Switchgear equipment to ensure that employees were warned about the hazards of arc flash. A Building Maintenance Technician sustained severe burns to the face, neck, and chest when an arc flash occurred as he worked near or in close proximity to energized electrical parts at 13,200 volts. Observed On or About 06/22/18
