29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    (a)  On or about 06/27/18 - 1424 Welti Road SE, Cullman, Alabama, employees engaged in roofing activities on a steep roof (10/12) were exposed to fall hazards of approximately 10 feet, 8 inches to the ground.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a) On or about 06/27/18 - 1424 Welti Road SE, Cullman, Alabama, employees were using a ladder to access the roof that was not extended 3 feet or more above the upper landing surface.
