On September 18, 2018, Jim Costello dba Jim Costello Farm & Industrial Painting was issued citations and the employer did not verify, through documents that each citation was abated.  Abatement documentation was not received for the following violations:      a.    Citation 1 Item 1a, and  b.    Citation 1 Item 1b, and c.    Citation 1 Item 2a, and d.    Citation 1 Item 2b, and e.    Citation 1 Item 3a, and f.     Citation 1 Item 3b.

On or about June 27, 2018, employees engaged in painting the roof of a barn were exposed to fall hazards up to approximately 25ft when accessing the roof by extension ladder and the ladder rails were not extended at least 3ft above the landing surface being accessed and the ladder was not secured.
