29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  1737 N. University Ave., Lafayette, LA (west side of building):  On or about June 28, 2018, two employees installing Perma Base (cement board) were working from the basket of an aerial lift approximately 15 ft. high without fall protection.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  1737 N. University Ave., Lafayette, LA:  On or about June 28,2018, the employer did not instruct employees in methods to control or eliminate potential fall hazards prior to working from an aerial lift.  The employees were working to install Perma Base (cement board) to the building at a height of approximately 15 ft.
