29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:    On or about June 27, 2018, at the jobsite an employee was observed standing on the guardrails of a lifting platform using it as a working platform which was not at least 18 inches wide.  This exposed the employee to the hazard of falling.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:    On or about June 27, 2018, employees were observed working from a steel personnel lifting platform mounted to a forklift truck and elevated to a height approximately 14 feet.  The personnel lifting platform was observed not secured to the lifting carriage and/or forks of the forklift truck.  Employees were exposed to fall hazards.
