On or about 6/26/2018 at 11665 Lewis Braselton Boulevard Braselton, GA, rear of building, employees performing bracket installation activities were exposed to fall hazards of 13-feet to the concrete pad below where the job-made mobile scaffold working surface did not have complete guardrails installed at the ends of the extended platform.

29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner: On or about 6/26/2018 at 11665 Lewis Braselton Boulevard Braselton, GA employees were exposed to fall hazards of 13-feet to the concrete pad below where the job-made scaffold could be moved. The employer failed to insure that all locking type casters were properly functioning or in use. Casters in use were not positively locked in that the braking mechanisms did not make contact with all respective casters or were not engaged from left to right at the: a. far left front caster plate did not make contact with the wheel when brake is engaged. b. second row front caster brake not engaged c. second row back caster brake not engaged d. third row rear caster plate did not make contact with the wheel when brake is engaged. e. far right front caster plate did not make contact with the wheel when brake is engaged.
