29 CFR 1926.501(b)(6): Each employee on ramps, runways, and other walkways was not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems:  a) 2900 Horizon Drive King of Prussia, PA, Roof: On or about June 22, 2018, employees were using a 12 inch wide scaffold stage as a ramp from the roof, approximately six to nine feet above a lower level, to transport debris to a ground-level dumpster without being protected from falling to that lower level through the use of guardrail systems.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) 2900 Horizon Drive King of Prussia, PA, Roof: On or about June 22, 2018, employees were performing roofing work on a commercial roof, approximately nine feet above grade at a 4 in 12 roof pitch, without utilizing any means of fall protection.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  a) 2900 Horizon Drive King of Prussia, PA, Worksite: On or about June 22, 2018, employees used an aluminum extension ladder to access a commercial roof that had multiple bent ladder rungs along the climbing surface, but had not been removed from service.
