29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  On or about June 29, 2018, employees accessed the platform of a ladder jack scaffold, which was on the climbing side, by climbing up the support ladders. The front face of the platform was more than 2 feet above the portion of the ladder immediately below.

On or about June 29, 2018, employees performing roofing activities from the roof of a residential home with a pitch of approximately 9:12 were not protected from falling approximately 21 feet to the ground below.     b. On or about June 29, 2018, employees performing roofing activities from the roof of a garage with a pitch of approximately 6:12 were not protected from falling approximately 7.8 feet to the ground below.
