As referenced by the note contained in 1926.602(d):   At a residential construction site located at 27204 E Oakhaven Dr., Greenwood, MO: Employees were exposed to struck-by hazards. Employees were permitted to operate a JLG 944E-42 (Serial # 0160013669) brand all terrain fork truck and had not been trained and certified on the proper operation of the truck.

Employees were operating a INDUSTRIAL AIR brand air compressor, with the guard for the belt and pulley missing.

29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected fofore each day's use for external defects, such as deformed or missing pins or insulation damage, and for indicationsof possible internal damage.  At a residential construction site located at 27204 E Oakhaven Dr., Greenwood MO: Employees were exposed to electrical shock hazards. The employer failed to protect employee from electrical shock in that equipment was being used without a ground pin. This was demonstrated when a MAKITA brand circular (Serial # 1735476Y) saw was connected to an extension cord and did not have a ground pin to ensure the path to ground was permanent, continuous and effective. In addition the same extension cord was damaged in multiple locations and had clearly not been inspected prior to use.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  At the jobsite located at 27204 E Oakhaven Dr., Greenwood MO:  Employees engaged in residential framing activities were exposed to an electrical shock hazard.  The employer failed to protect employees from electrical shock hazards by allowing the use of flexible cords and cables with no strain relief. This was demonstrated when a MAKITA brand circular (Serial # 1735476Y) saw was connected to an extension cord and was found to have strain relief at the end connecting to the extension cord.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  At the worksite located at 27204 E Oakhaven Dr., Greenwood MO:  Employees engaged in residential framing activities were exposed to a fall hazard.  This was demonstrated when employees were using a JLG 944E-42 (Serial # 0160013669) brand all terrain fork truck equipped with a working platform to place roof decking. The platform was not protected on it's sides that could have resulted in an employee falling 10.5 feet to the ground below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    At the residential construction site located at 27204 E Oakhaven Dr., Greenwood, MO 64064:  Employees were exposed to fall hazards. Employees were performing framing operations at an elevated height of 10.5 feet without any means of fall protection. Employees were observed crawling along the trusses and roof decking without any mean of personal fall protection or fall protective systems.
