At a residential construction site located at 27204 E Oakhaven Dr., Greenwood MO: Employees were exposed to electrical shock hazards. The employer failed to protect employee from electrical shock in that equipment was being used without a ground pin. This was demonstrated when a MAKITA brand circular (Serial # 1735476Y) saw was connected to an extension cord and did not have a ground pin to ensure the path to ground was permanent, continuous and effective. In addition the same extension cord was damaged in multiple locations and had clearly not been inspected prior to use.
