29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) Site:  In the facility at 11 Industrial Drive, South Hadley, MA 01075 on or about 11/26/2018 and at times prior portable machine guards for the Bridgeport milling machines designated BP 1, BP 2, BP 3 did not protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.
