29 CFR 1926.404(f)(3)(ii): Vehicle-mounted generators. Under the following conditions the frame of a vehicle may serve as the grounding electrode for a system supplied by a generator located on the vehicle:  a.    On or about July 2, 2018, in the tool trailer, a portable generator used as a vehicle mounted-generator was not grounded while supplying power for employees using power hand tools, exposing them to the hazard of electric shock.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a.    On or about July 2, 2018, on the 6:12 slope roof, an employee installing truss framing and roof sheathing was exposed to a 10 feet to 13 feet fall hazard in that he was not protected by a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.    On or about July 2, 2018, at the front of the house, the roof access ladder did not extend at least 3 feet above the roof eave, exposing an employee to a 10 feet fall hazard.

29 CFR 1926.1053(b)(7): Care was not exercised in placing, lashing, or holding a ladder with slip-resistant feet that was used on a slippery surface:  a.    On or about July 2, 2018, at the front of the house, the feet of the roof access ladder were not spiked into the sandy ground where the ladder was used by a framing employee.
