29 CFR 1910.22(d)(2): The employer must ensure that hazardous conditions on walking-working surfaces are corrected or repaired before an employee uses the walking-working surface again.  If the correction or repair cannot be made immediately, the hazard must be guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired.    Rural King Supply store no. 55 in Plano, IL, mezzanine storage: on or about July 3, 2018, employees working in the main walkway were exposed to trip and fall hazards because identified plywood floor holes were not repaired timely or temporarily guarded prior to permitting employees to use the space.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    Rural King Supply store no. 55 in Plano, IL, mechanic's shop, relocatable power tap - on or about July 3, 2018, subject energized relocatable power tap was not directly connected to a permanently installed branch-circuit receptacle; was connected in-series (daisy chained) to other extension cords; was permanently secured to a work table; and, was used as a substitute for fixed wiring, all prohibited by the manufacturer's instructions and the listing standards of a nationally recognized testing laboratory (Underwriters Lab). Employees using power tools and other connected equipment were exposed to electrical hazards (fire and burns).    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  Rural King Supply store no. 55 in Plano, IL, mechanic's shop, extension cords: on or about July 3, 2018, employees were exposed to fire and electric shock hazards where the connected in-series extension cords were used for permanently powering tools, equipment, and overhead lights.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1904.40(a): Basic requirement. When an authorized government representative asks for the records you keep under Part 1904, you must provide copies of the records within four (4) business hours.    Rural King Supply store no. 55 in Plano, IL - on or about July 3, 2018 and July 13, 2018, the employer failed to provide requested copies of required work-related injury and illness records (Forms 300, 300A, and 301 for years 2015 - 2018) to OSHA within four business hours from the initial request onsite, and followed up by a written request due July 13, 2018.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
