29 CFR 1926.451(e)(5)(i): Ramps and walkways 6 feet (1.8 m) or more above lower levels did not have guardrail systems that comply with subpart M of this part -- Fall Protection:      On or about July 3, 2018 the employer failed to ensure that employees were protected from falling off the walkway from the rear roof area to the scaffolding. Employees gained access to the scaffolding by a wooden plank placed between the scaffolding and building at a height of approximately 11 feet.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:       On or about July 3, 2018 the employer failed to ensure that employees working from a scaffolding system were protected from falling to the ground below. Employees were observed working from a scaffold approximately 12 feet in height without conventional fall protection measures taken. No guardrails were in place on the scaffold.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems;      a) On or about July 3, 2018 the employer failed to ensure that employees were protected from falling to the ground below while working from the roof area of the back of the facility. Employees were observed working from the roof area which had a height of approximately 7 feet to the ground below.           b) On or about July 3, 2018 the employer failed to ensure that employees were protected from falling to the ground below while working from the roof area of the back of the facility. Employees were observed working from the roof area and standing on a bushel of foam boards which had a height of approximately 11 feet to the ground below.      c) On or about July 3, 2018 the employer failed to ensure that employees were protected from falling to the ground below while working from the roof area of the main structure of the facility. Employees were observed working from the roof area and standing on and near the edge  which had a height of approximately 15 feet to the ground below.

The ladder was in the closed position, placed against a wall, and used to ascend onto a roof area.
