Employees gained access to the scaffolding by a wooden plank placed between the scaffolding and building at a height of approximately 11 feet.

Employees were observed working from a scaffold approximately 12 feet in height without conventional fall protection measures taken. No guardrails were in place on the scaffold.

Employees were observed working from the roof area which had a height of approximately 7 feet to the ground below. b) On or about July 3, 2018 the employer failed to ensure that employees were protected from falling to the ground below while working from the roof area of the back of the facility. Employees were observed working from the roof area and standing on a bushel of foam boards which had a height of approximately 11 feet to the ground below. c) On or about July 3, 2018 the employer failed to ensure that employees were protected from falling to the ground below while working from the roof area of the main structure of the facility. Employees were observed working from the roof area and standing on and near the edge which had a height of approximately 15 feet to the ground below.
