29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet (1.8m) or more above the lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems:    At the jobsite located at 1809 Smithmoor Street, Wichita, Kansas, employees working from the edge of the roof were exposed to falls of approximately 11 feet or more in that no fall protection system was used.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, was not provided to assist employees in mounting and dismounting the ladder:    At the jobsite located at 1809 Smithmoor St., Wichita, Kansas, employees using the ladder to access the roof were exposed to falls to grass of approximately 11 feet in that the ladder did not reach three feet above the upper surface and the ladder was not secured at its top.
