29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: On June 29, 2018, on a lot located at the corner of NE Todd George Rd and NE Riley Dr., Lee Summit, MO., an employee was exposed to serious injuries from a fall hazard, in that the worker was observed standing on several sheets of plywood that were stacked on the forks of an elevated JLG Lift. There was an approx. 11 ft. fall to the ground and no Fall Protection was in use.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On June 29, 2018, on a lot located at the corner of NE Todd George Rd and NE Riley Dr., Lee Summit, MO., employees were exposed to serious injuries from a fall hazard, in that the workers were engaged in framing activities more than 6ft. above lower levels without Fall Protection being used.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii) On June 29, 2018, on a lot located at the corner of NE Todd George Rd and NE Riley Dr., Lee Summit, MO., the employer did not ensure that all employees exposed to fall hazards while engaged in framing activities, were trained to recognize and know when fall protection is needed and required. Workers were exposed to falls greater than 6 ft. above lower levels without the benefit of Fall Protection.
