29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):     (a)  Don Meny's Roofing Inc. Located at 800 Pikes Peak Ct. Louisville, CO 80027: On and preceding 7/7/2018, the employer did not ensure that employees engaged in residential roofing activities 6 feet or more above lower levels were protected by guardrail systems, safety net systems, or personal fall arrest systems.  Two employees were installing shingles and trim on the roof of a two-story residential home with a steep pitch without the use of fall protection.  This exposed employees to falls from height.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed:     (a)  Don Meny's Roofing Inc. Located at 800 Pikes Peak Ct. Louisville, CO 80027: On and preceding 7/7/2018, The employer did not ensure that ladders used to access an upper landing surface had side rails extended at least three feet above the upper landing surface.  Employees were using an extension ladder to access the roof with approximately 1'6" extending above the landing surface.  This exposed employees to falls from height.
