At a worksite located at 168 Ewing Way, Belton, Missouri- Employees were exposed to a fall hazard of approximately 11-13 feet. Employees were accessing a working surface by way of a portable ladder whose side rails did not extend at least 3 feet above the upper landing surface being accessed.
