At a worksite located at 168 Ewing Way, Belton, Missouri- Employees were exposed to a fall hazard of approximately  11-13 feet. Employees were accessing a working surface by way of a portable ladder whose side rails did not extend at least 3 feet above the upper landing surface being accessed.

29 CFR 1910.28(b)(1)(i):   The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:    At a worksite located at 168 Ewing Way, Belton, Missouri- Employees engaged in tree trimming activities were exposed to a fall hazard of 11-13 feet from the upper level to the surface below. A complete guardrail system or other means of fall protection was not provided.

At a worksite located at 168 Ewing Way, Belton, Missouri-  Employees were exposed to serious struck by injuries in that the employer did not assess the workplace to determine if hazards were present, or likely to be present, which would necessitate the use of personal protective equipment, for employees performing commercial tree trimming activities, high visibility vest were not supplied to grounds crew members while working around trucks and other mobile equipment.
