29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about June 29, 2018, employees shingling the roof of a new residential home located at 24850 Eagle Pointe Columbia Station, OH 44028 were exposed to a fall approximately 9.9 feet high with a 7:12 pitch. The employer did not protect the employees from falling through the use of fall protection systems/devices.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about June 29, 2018, during the installation of roofing material at a job site located at 24850 Eagle Pointe Columbia Station, OH 44028; employees were climbing a portable ladder in order to access the roof and the employer did not ensure the ladder was not extended three feet above the landing surface. Employees were exposed to a fall height of approximately 9.9 feet.
