29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:    (a) Facility, Store/Gasoline Station: On or before July 5, 2018, the employer bolted and plated a rear exit door closed at the facility. An employee was overcome by heavy smoke, carbon monoxide, and heat when they were unable to escape through the rear exit door after a fire occurred.
