29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    a) Car Wash located at 3550 Brooks Street, Missoula, MT: On or about July 11, 2018 and times prior, employees were exposed to catapult hazard and subsequent fall while working from a JLG aerial lift, model 400S, and serial 0300153174 from a height of approximately 10 feet, without using personal fall protection.
