29 CFR 1926.451(e)(1): Portable ladder, hook-on ladder, attachable ladder, stair tower, stairway-type ladder, ramp, runway, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used to access scaffold platform more than 2 feet above or below a point of access:    A) 25 Hillside Ave Metuchen, NJ 08840    Employees were accessing the upper levels of the scaffold by climbing the exterior of the scaffold. No safe means of access was provided.    Occurred on or about 07/12/18
