29 CFR 1926.1053(b)(13): The top or top step of a stepladder shall not be used as a step. a. On or about July 12, 2018 at 1317-1319 Prairie Lake Circle Neenah, WI, the employer did not ensure that an employee using a folding stepladder did not step on the top step and cap when unloading pre-fabricated walls from a trailer. The employee was exposed to fall hazards of approximately six feet while attaching chains to pre-fabricated walls on a flatbed trailer that were part of a residential construction project.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. a. On or about July 12, 2018 at 1317-1319 Prairie Lake Circle Neenah, WI, the employer did not ensure that an employee was wearing a protective helmet to prevent possible injury from impact. The employee was exposed to struck-by injuries from lift chains connected to a crane that he was attaching to pre-fabricated walls used in a residential construction project.
