Employees were re-roofing a 4/12 pitch roof of a multiple story home. Two employees were not protected by guardrail systems, safety net systems, or personal fall arrest systems. The employees had not been provided effective fall protection training. This condition exposed the employees to approximate 8.2 foot and 13.5 foot fall hazards.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder: (a) Wes Brown Roofing Inc, at 2542 Franklin Avenue, Louisville, CO: On 7/11/18, the employer did not ensure that ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder. Employees were re-roofing a 4/12 pitch roof of a multiple story home. The employees utilized an aluminum extension ladder to access the first story roof and the second story roof. The ladder's feet rested upon grass and soil. The right side ladder rail rested approximately eight inches from the end of the roof edge. The ladder was not secured to prevent accidental displacement. This condition exposed the employees to approximate 8.2 foot and 13.5 foot fall hazards.
