29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) JSP Enterprises LTD at 3970 Holly Street Denver, CO 80207: On or before July 12, 2018 the employer did not ensure the point of operations on the PA 90 machine were properly guarded to prevent employees from having any part of their body in the danger zone during operation. The distance from the steel paddles to the guards on the extrusion bin was approximately 4 inches, however the width of the gaps between the safety bars was approximately 1 1/2 inches. The maximum gap in the safety guarding that is allowed for a distance of 4 inches is 5/8 inches.  Also, 2 safety bars were missing on each side of the guards which created two 3 inch gaps in the safety guard. This hazard exposes employees to amputation hazards.    b) JSP Enterprises LTD at 3970 Holly Street Denver, CO 80207: On or before July 12, 2018 the employer did not ensure the point of operations on the PA 90 machine were properly guarded to prevent employees from having any part of their body in the danger zone during operation. The distance from the steel paddles to the guards on the mixing bin of the PA 90 machine was approximately 5 inches, however the width of the gaps between the safety bars was approximately 1 1/2 inches. The maximum gap in the safety guarding that is allowed for a distance of 4 inches is 5/8 inches. This hazard exposes employees to amputation hazards.    c) JSP Enterprises LTD at 3970 Holly Street Denver, CO 80207: On or before July 12, 2018 the employer did not ensure the point of operations on the PA 90 machine were properly guarded to prevent employees from having any part of their body in the danger zone during operation. The mixing bin on the PA 90 machine has a contact style system that shuts down the rotation of the steel paddles. However, there have been several instances during he past year when the system has intermittently malfunctioned and not stopped the rotation of the paddles while the guard was open. This hazard exposes employees to amputation hazards.
