On or about 6/19/2018, in the ground maintence building, on the work bench, employees were exposed to amputation hazards, in that, the rotating blades on the cutting unit for the Toro 3100D mower were not guarded while the backlapping machine turned the blades and employees were applying a lubricant compound to the blades as they rotated.

On or about 6/19/2018, at the jobsite, in the ground maintenance building, the floors, work bench, storage shelves and outside area were cluttered and dirty, in that, there were bags of fertilizer, stacks of old batteries, spare mower parts, buckets of various oils, and fuel cans scattered around the shop and the shelves & work bench were cluttered with aerosol cans, loose tools, and various repair parts.
