29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l): At the commercial construction jobsite located at 7651 W. 159th St. in Overland Park KS: Employees were exposed to struck-by hazards. Workers engaged in installing masonry onto the exterior of a new building (Blue Valley Hospital) with the use of a CAT Tele-handler all terrain fork truck were exposed to struck by hazards. The employee operating the PIT was not certified to operate the equipment.

29 CFR 1926.450(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports: At the commercial construction jobsite located at 7651 W. 159th St. in Overland Park KS: Employees were exposed to fall hazards. A worker engaged in installing masonry onto the exterior of the new building (Blue Valley Hospital) with the use of scaffolding was exposed to fall hazards of approximately 12 feet to the ground below. The worker was exposed in that the scaffold was not completely planked.
