29 CFR 1926.452(w)(6)(v): Employees were allowed to ride on scaffolds while employees were on parts of the scaffold which extend outward beyond the caster, wheels, or other supports:  (a): Mobile Scaffold in Driveway: Employees working on the scaffold side brackets and the scaffold main deck were exposed to fall hazards of approximately four feet and seven feet while riding the mobile scaffold that was being moved, on or about July 16, 2018.   There is no abatement certification or documentation required for this item.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a): Exterior Ledge Over Driveway: An employee working on a ledge was exposed to fall hazards of approximately 13 feet. An employee was performing stucco work on a ledge and was not protected from falling by any means, on or about, July 16, 2018.  There is no abatement certification or documentation required for this item.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access.  (a): Mobile Scaffold in Driveway: Employees were exposed to fall hazards of approximately seven feet while accessing a mobile welded frame scaffold. Employees were not provided with sufficient means to access the scaffold and were gaining access by climbing the frame, on or about July 16, 2018.   There is no abatement certification or documentation required for this item.  Excellent Stucco LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number 1245971, citation number 1, item number 3 and was affirmed as a final order on September 25, 2017, with respect to a workplace located at 14 Van Wagoner Drive, Englewood Cliffs, NJ 07632.
