29 CFR 1926.502(i)(2): All other covers were not capable of supporting, without failure, at least twice the weight of employees, equipment, and materials that may be imposed on the cover at any one time: a) 6 Memorial Beach Dr., Webster, MA: On or about June 12, 2018, the cover over an access pit to a lower level did not support the weight of the employee walking on the cover, and its failure led to him falling to the ladder landing, more than 15 feet below the cover. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).

29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: a) 6 Memorial Beach Dr., Webster, MA: On or about June 12, 2018, the cover over an access pit to a lower level was not secured to prevent displacement and its displacement led the employee to fall through the opening to the ladder landing , more than 15 feet below the cover. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).
