29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedure) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6):    a) Courtyard Area- On or about July 5, 2018, an employee was exposed to machine hazards associated with moving parts and pinch point hazards during the maintenance and servicing of the Glue and Roll Coat Lines Oxidizer 1097.  The employer failed to implement energy control application steps as lockout tagout (LOTO) devices were not applied to the electrical disconnect, gas supply valve, and three pneumatic valves [per the 1910.147(d)(4) requirements.  As a result, the remaining applicable energy control elements, involving dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew:  a) Courtyard Area- On or about July 5, 2018, the employer did not ensure that employees utilized a lockout procedure that provided employees a level of protection equivalent to that provided by the implementation of a personal lockout device to protect them from the unexpected energization of the equipment when work was performed by a group on the Glue and Roll Coat Lines Oxidizer 1097.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures:    a) Courtyard Area- On or about July 5, 2018, the employer and the outside servicing personnel of Advanced Technology Services, Inc. as well as BorgWarner Transmission Systems LLC, did not inform one another of their lockout tagout procedures.  All three companies had procedures, but none of the three were utilized and it contributed to confusion among employees of the responsibilities with the application of lockout on the Glue and Roll Coat Lines Oxidizer 1097.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
