29 CFR 1926.403(b)(2): Listed, labeled, and/or certified equipment was not installed and/or used in accordance with instructions included in the listing, labeling, and/or certification: a) 250 Whalom St., Lunenburg, MA: On or about July 16, 2018, an employee was observed using a wet tile saw which was powered through a GFCI device which was plugged into an extension cord to receive its electricity, which is contrary to the instructions printed on the device.
